The Santa Rosa Post Acute Mission

Recovery asks a great deal of a person, and it rarely follows a predictable schedule. Our mission at Santa Rosa Post Acute is to make that process safe, as comfortable, and as steady as it can be. We do that through a resident-centered approach built on compassionate bedside nursing, specialized wound care, and coordinated rehabilitation. Our interdisciplinary team works closely together to develop individualized care plans focused on healing, restoring independence, and helping each resident achieve the best possible outcome. We rely on evidence-based practice and continuous quality improvement to hold that standard, and our five-star Quality Measures rating reflects it. We also work in step with your primary physician, so the care you receive here connects to the care that came before it. Whatever brings you to us, the goal is the same: helping you return to the life you know, with as much strength and independence as possible.

Care here is delivered by people who work as one team rather than separate departments. Our nurses, therapists, dietitians, social services staff, and leadership meet around the same care plan and the same resident. That coordination is what keeps an individualized plan holding together day to day. Get to know the departments that will be part of your stay.

Leadership

Our administrator, admissions, and marketing leaders set the tone for the building, supporting every department so residents and families receive consistent, well-coordinated care.

Therapy / Rehabilitation

Our therapy team delivers physical, occupational, and speech therapy, working from individualized plans that help residents rebuild strength, mobility, and independence.

Nursing / Clinical Staff

Our nursing team provides attentive, hands-on care around the clock, with close monitoring and compassionate support tailored to each resident’s individual needs.

Social Services & Activities

Our social services and activities teams support the non-clinical side of a stay, from care and discharge planning to daily programs that keep residents active and socially connected.

Dietary Services

Our dietary team and registered dietitians prepare nutritious meals that meet each resident’s dietary and therapeutic needs, according to physician orders and personal preferences.

Environmental Services

Our environmental services team keeps the building clean, safe, and welcoming, so residents, families, and visitors feel comfortable from the moment they walk in.

Today, I am leaving SRPAC after a month’s stay. For the most part, it has been a pleasent experience. The food is fairly good and the care, especially PT/OT and Social Workers, are excellent. Most of my CNA’s went above and beyond with a few exceptions. Thanks for taking such great care of me,
