If I could give zero stars, I would. Santa Rosa Post Acute feels severely understaffed, poorly maintained, and disorganized. My father was repeatedly not helped in a timely manner with basic hygiene and cleaning needs, despite multiple requests. Staff often seemed to ignore or delay even simple care tasks. Communication is extremely poor and follow-through feels inconsistent at best. The facility itself also looks neglected, with worn-out furniture and an overall sense of being underfunded. During my visits, I personally saw cockroaches, which was shocking and unacceptable in a medical care setting. Even basic logistics like parking are a hassle due to very limited spaces. Overall, this place does not meet basic expectations for patient care or cleanliness. I would strongly advise looking elsewhere.

If you’re looking for a facility that truly cares, choose Santa Rosa Post Acute. The attention to detail and consistency in care are above board. The staff is attentive, compassionate, and genuinely invested in their residents’ well-being. Highly recommend.

I’ve been a resident twice. The staff and nurses are professionals If I have an issue it is quickly resolved.Sometimes the system is a challenge. But everyone here perseverance and treated me with respect and accomodations to make me comfortable as possible. Out of the employees are hard working with the services they provide. My thanks goes to the nurses, CNA that care for us deeply.

Hello SR Post Acute, My family wants to thank you for all of the great care that our 96 year old father has received at your facility. Thanks to the wonderful PT, nurses, chefs, social workers and everyone that pitches in our dad has recovered enough to go on outside visits with us to restaurants and to visit old friends. This would have never been possible without the incredible teamwork that it takes to help elderly people recover! Thanks for the great care and thanks for making the difference in our dad’s life!🙏🏻

Staff not only took care of a trip to the hospital expeditiously , but helped me with some furnishings to make my sister more comfortable upon her return. Banner day.

This place isn’t great. Walking in you smell urine smell. The staff isn’t in a rush to help people. Waiting two hours for a pain medication while in pain speaking for a patient is completely unacceptable! The room of a patient is sticky. Bed pads are not provided here. I have brought some. But this place should provide them upon request. Physical and occupational seems to be good here. I cannot recommend anyone coming here as a patient. Do your research.

My brother was put in SRPA from Kaiser, after a terrible injury. SRPA has been so helpful to him and as his only living relative and 5 hrs. away, I visit as often as I can. Everyone I encounter during my visits are pleasant, respectful and professional.

We had to call the police for a welfare check on my 80 year old neighbor because they don’t Answer the phone it doesn’t even ring it just music playing after the police went there we finally got ahold of our neighbor. Another disturbing note I asked to speak to my neighbor in the hospital she has no friends or family I expressed that to the receptionist she flat out said oh u don’t need to talk to her u need to talk to her social worker furthering blocking me from comforting my elderly neighbor or even checking on her. Look Answer the phone and respect when people are trying to reach out to love ones should not take the police. The good reviews must be fake on here!

Play sucks after the second time around
My need to stay and be cared for as I healed , great nurses and staff the physical therapy team really did wonders with me After being uncomfortable about having to need the help it really was a good experience and helped me very much

In February 2025, my 95-year-old father transitioned to Santa Rosa Post Acute after spending time in a San Francisco hospital due to an infection related to his pacemaker. He required full-time nursing care, physical therapy, prepared meals, assistance with mobility, and personal care support. I want to express our heartfelt satisfaction with the incredible staff at Santa Rosa Post Acute. I wholeheartedly recommend this excellent facility to anyone with a loved one needing nursing home care. Upon my father’s arrival, the staff physician immediately contacted us to inform us about his condition and discuss his treatment plan. As a veteran, my father was also offered assistance with transportation to his VA appointments. He completed his physical therapy program and focuses on maintaining daily walks with staff support and gaining weight. We needed to call the front desk for help with my dad’s iPhone issues, and the staff was always friendly and understanding, assisting him with his tech questions. My dad enjoys the food here more than at all the places he’s been to, and he has even gained 3 pounds! We’ve had many opportunities to speak with the staff, including the physician, social worker, director, director of nursing, cafeteria team, TV repair crew, and daily support team. Our experiences have been overwhelmingly positive, and we have found everyone incredibly caring and supportive- not just for their patients but for each other. The team at Santa Rosa Post Acute truly goes above and beyond to meet the needs of their patients and families. They are always ready to listen to your concerns and dedicate their time to help find solutions. In this challenging world of negative reviews, our family has prioritized effective communication and addressing difficult situations before they lead to unwarranted negative feedback. I was a patient at Santa Rosa Acute for three months. I received really good care from Physical therapy, Nurses, Drs.,CNA’S, the Administration, Housekeeping. Everyone treated me with a alot or respect. Its very hard being in a facility. Thank you very much

The Director of Nursing kept me informed with my dad’s stay here. She facilitated his recovery and discharge. They truly care about their patients and it shows. This facility has been one of the best I’ve seen

My Grand father recently stayed at Santa Rosa Post Acute for skilled nursing care, and overall, our experience was quite positive. The staff members were friendly and approachable, which made a tough situation a little easier. They seemed genuinely concerned about the well-being of the patients and were always ready to answer our questions. The facility itself was clean and well-kept, which added to the comforting atmosphere. My Grand father appreciated the activities offered to keep patients engaged, and it was nice to see them participating and socializing with others. One thing I noticed was how attentive the nursing staff was to individual needs. They took the time to ensure that my Grand father was comfortable and understood their care plan. Overall, I would recommend Santa Rosa Post Acute for anyone looking for skilled nursing care. They made a difficult time much more bearable, and I’m thankful for their support.
